A typical shift tends to involve continuous one-to-one care of a complex patient — vent management, multiple drips, frequent assessments and interventions, family teaching, and detailed documentation across the entire shift. Senior nurses on multi-shift cases often serve as the de facto lead — orienting new staff, escalating concerns, coordinating with the agency.

Coordination tends to be with the patient and family, supervising RN or case manager, physicians, and other shifts handing the patient off. The hardest part is often the relational complexity of long single-family engagements — boundaries that need to hold across years, family dynamics that surface in a home setting, the deep familiarity that makes objectivity hard. Burnout in private duty looks different from hospital burnout.
